Thank you for your interest in CMMS. In order to reserve your child’s placement at CMMS, please complete the below enrollment form and submit the $200 non-refundable enrollment fee. Following your submission, you will receive an email confirmation of enrollment including instructions on how to submit your tuition deposit to complete the enrollment process.
